Description of activity
The Children's Continuing Care Team provide healthcare for children who have a range of conditions that would otherwise need hospital treatment or residential care.
The team consists of experienced children’s nurses who provide an assessment service for those children who are referred to children’s continuing care. The child is assessed using the National Framework for Children and Young People’s Continuing Care, March 2010.
Following agreement of their eligibility at the Joint Funding Panel, a care package is sourced/provided and monitored by the team. Following the initial assessment if the child meets the criteria, the child’s care package will then be reviewed every 3-12 months according to the level of need.
The team provides:
Assessment of eligibility
Provision of care package as commissioned
Monitoring of care package
Provision of nursing equipment and supplies
Referrals are accepted for any child under 18 who is thought to meet the children’s continuing care criteria, and is registered with a Richmond GP.
Any child who is not registered with a Richmond GP is not eligible for an assessment.
Children who do not meet the criteria are not eligible for the service, but will be referred to the local children’s community nursing team if they have nursing needs.
